Backed by more than five decades of combined clinical experience, the practice is committed to providing professional, skilled, and compassionate care, coordinating services, and creating treatment plans to address the varied health needs of the community. 

Our practice provides patient-focused services and is open to those who have lost access to primary care due to clinic closures or facilities that offer care exclusively on a concierge basis.

Update Insurance Information

Contact us or your health plan's member services to confirm if we are in-network and authorized to serve as PCP (Primary Care Provider) for your health plan.

Obtain and complete any required PCP designation forms, or update your selection through your health plan’s member portal, providing Dr. Amir Alizadeh’s full name, practice address, and National Provider Identifier (1942333877) if requested.

After submission, confirm the effective date of the change from the health plan that your member record is updated, designating Dr. Amir Alizadeh as your PCP.

Sign Up for the Patient Portal

You may optionally sign up for the Patient Portal for secure, convenient access to your health information and care tools to view medical records, and test results, and view summaries online; request prescription refills; schedule or change appointments and receive reminders; pay bills and review insurance information; and complete forms prior to visits.

Sign up for to the Patient Portal found on the upper right of the menu and create an account and register your name as it appears on your ID and insurance information. If you have questions about the Patient Portal, contact the office for assistance.  

Before an Appointment

Prepare for the phone consultation: list symptoms, medical history, current medications, allergies, and any questions for Dr. Amir Alizadeh.

Call us at 401-354-4400 and confirm whether an in-person appointment is recommended and ask to book your appointment date and time under your preferred contact details and to send confirmation via email or text.

Before scheduling an in-person appointment, confirm that Dr. Amir Alizadeh is listed as in-network with your insurance carrier or health plan and ask us for any out-of-pocket costs to prepare.

If this is your first consultation with us, please complete the consent forms found under Services > Online Forms before your appointment. These forms gather essential medical history and permissions needed to proceed with care. If you have questions while filling them out, contact the office for assistance.

On Your Appointment

Prepare documents for the appointment day: photo ID, insurance information, referral forms (if given), a current medication list, recent test results, and any relevant medical records.

On the appointment day, arrive 10–15 minutes early with all documents, wear a mask if you have respiratory symptoms, and have a written list of priorities and questions for Dr. Amir Alizadeh.

Follow the nurse's instructions for basic screening, including blood pressure, height, and weight measurements, while waiting to see the doctor.

When receiving guidance from Dr. Amir Alizadeh, listen carefully and reserve all questions until after your conditions and treatment options are discussed.

When receiving prescriptions from Dr. Amir Alizadeh, confirm preferred pharmacy, medication name, dose, frequency, duration, and any side effects or interactions to watch for.

Follow all diagnostic orders given by Dr. Amir Alizadeh: schedule lab work, imaging, or specialist referrals promptly and bring all printed results on your follow-up consultation.

After Every Appointment

Set calendar reminders for follow-up appointments and any time-sensitive diagnostic tests ordered by Dr. Amir Alizadeh.

Use the Patient Portal to review consultation notes, test results, and diagnostic orders or message the staff for clarifications or non-urgent questions.

You may manage prescription refills through the Patient Portal or by contacting the clinic. Allow several business days for refill requests and provide current pharmacy information.

If symptoms change or worsen, contact the office promptly and follow emergency guidance as advised.
